10 Completing the Placement Proposal Form
When you’ve identified a suitable placement provider, you are required to complete a placement proposal that is submitted to the module leader.
You cannot begin placement until the module leader has confirmed that you have approval to do so.
The proposal, which is completed online, requires you to respond to the following prompts. You should collate all necessary information from the provider before completing the online form.
Note that the form does not allow you to save a partially-completed entry.
You confirm that you have discussed the possibility of a placement with the provider, and that they have responded positively.
You enter your student registration number.
You enter your surname and first name.
You enter your university email address.
You enter the name of the placement provider.
You enter their postal address, which should be the location where you will be based (you should find this out prior to completing the form).
You provide a contact telephone number for the provider.
You enter the name of the key contact at the provider – this will be the person named as your ‘placement supervisor’. Again, you should find this out prior to completing the proposal.
You enter an email contact address for this individual.
You enter the start and finish dates for your proposed placement. These should be between October 2024 and April 2025.
You should state your reasons for selecting this placement provider. Specifically, how will the experience gained in placement relate to your personal development needs and future career aspirations?
You should provide a description of the kinds of activities and duties you will undertake.
You confirm that you have an appropriate Strathclyde PVG certificate (usually completed as part of B1221 in Year 2).
When the proposal is submitted, the module leader will contact the individual named at step (8) above.
If they respond positively to you completing your placement with them, the module leader will then contact you directly (using the email address provided at step (4) above) and advise you to liaise with the provider to agree a start date.
